Boarding/Daycare Consent

Sam & Danni’s Pet Resort & Spa, Inc.

I entrust Sam & Danni’s Pet Resort & Spa, Inc. with the care of my pet(s) for his/her boarding stay and agree to all of the following items:

1. All dogs/cats/other boarded, handled, or cared for by Sam & Danni’s without liability on our part for loss or damage from disease, death, running away, theft, injury to persons, self inflicted injury due to behavior problems, other dogs, or property by said dog/cat/other, or other unavoidable causes, due to diligence and care having been exercised.

2. I understand that boarding my dog may make my dog susceptible to contract tracheobronchitis (canine cough). Sam & Danni’s requires current vaccinations and does not let any dog board that is obviously sick with canine cough. Disinfecting is done daily and air exchanging is constant. I will not hold Sam & Danni’s liable of any of the cost of treatment in the event my dog contracts canine cough. Literature is available upon request.

3. The dog/cat/other is not to be taken off the premises except by the owner. If you need to arrange for someone other than yourself to pick up your pet, you will need to pre-authorize for pick-up. If you have someone else pick up your pet, Sam & Danni’s is not responsible should anything happen to your pet once it leaves our building. Your credit card will be billed unless you make other arrangements. Visitors other than the owner must be set up in advance, and will not be allowed to take pet(s) off premises.

4. If the dog/cat/other becomes seriously ill or injured, the owner or emergency contact will be notified at once. If the owner/emergency contact does not inform immediately regarding measures to be taken, or if the state of the pet’s health demands quick action, I, the owner or agent of the owner, give Sam & Danni’s the right to make judgement calls regarding the health of my pet. Such expenses, shall be paid promptly by me the owner or agent of the owner.

5. If you fail to pick up your pet two weeks after your scheduled departure date, we will consult your emergency contact about your whereabouts. In the event of pet abandonment, Sam & Danni’s reserves the right to place your pet in the care of a pet adoption agency. HOWEVER, in the event of an accident happening to you or changes in your plans, Sam & Danni’s must be notified by you or an acting agent to provide further accommodations.

6. If your pet is presented for boarding and is found to have fleas and/or ticks, it will be bathed at the owner’s expense.

7. I agree to pay for any extra services requested by myself or the person responsible for my pet’s arrival. You will be required to initial the check-in sheet for these services. (Walks, Playtimes, Packages, Bathing, Grooming, Fun Kong’s, etc.)

8. Sam & Danni’s will not be responsible for lost articles such as toys, leashes, collars, chewed items, etc. Sam & Danni’s will not be responsible for obstructions due to pet eating toys, bones etc. Please do not bring your pets “favorite items”.
